Travel Directions to the Home

We provide directions for those who drive, as well as those who use public transportation. Please see below.

From Eastern Long Island – Southern State Parkway West onto Belt Parkway West. Take Exit 17W (North Conduit), continue on North Conduit to Linden Blvd exit on your left. Stay on the service road (right lane) until you pass a McDonalds – Drew Street. Turn right on Drew Street, a left on Dumont Avenue and a right at Forbell Street. Entrance to the Home is on your left.

From Staten Island – Verrazano Bridge to Belt Parkway East. Take Exit 17 (Cross Bay Blvd) and turn left. From the right lane turn right on North Conduit West (small sign). Continue on North Conduit to Linden Blvd exit on your left. Stay on the service road (right lane) until you pass a McDonalds – Drew Street. Turn right on Drew Street, a left on Dumont Avenue and a right at Forbell Street. Entrance to the Home is on your left.

From Manhattan – Queens Midtown Tunnel onto Long Island Expressway (495) east. Take Woodhaven Blvd to Sutter Avenue (after 109 Street). Follow Sutter Avenue until you pass the North/South Conduit. Turn left at Forbell Street and proceed for 2 blocks. Home is on your right.

From Brooklyn – Atlantic Avenue to South Conduit (veer right where the two streets meet), proceed to Forbell Street (shortly after the Pitkin Avenue traffic light), turn right, 2 blocks to the Home.

From Westchester – Hutchinson Parkway to Whitestone Bridge and keep right onto the Wan Wyck Expressway (going toward Kennedy Airport). Follow the Belt Parkway West/Verrazano Bridge route. You may proceed on the North Conduit to Linden Blvd. exit. Stay on the service road (right lane) until you pass a McDonalds – Drew Street. Turn right on Drew Street, a left on Dumont Avenue and a right at Forbell Street. Entrance to the Home is on your left.

From New Jersey – George Washington Bridge to the Whitestone Bridge. Keep right onto the Wan Wyck Expressway (going toward Kennedy Airport). Follow the Belt Parkway West/Verrazano Bridge route. You may proceed on the North Conduit to Linden Blvd. exit. Stay on the service road (right lane) until you pass a McDonalds – Drew Street. Turn right on Drew Street, a left on Dumont Avenue and a right at Forbell Street. Entrance to the Home is on your left.

By Bus:

B13, B14, B15 and B20 to Elderts Lane and Linden Boulevard. Proceed North to Elderts Lane one block to Dumont Avenue and you will see the Nursing Home. (Entrance is on Forbell Street between Dumont and Blake Avenues.)

By Train:

IRT 7th Avenue Line:3 train to New Lots Avenue (last stop). Leave station at New Lots & Livonia Avenues and take B15 bus to Elderts Lane. Proceed with bus directions above. The nearest B15 bus stops are at Elderts Lane & Linden Boulevard.

IND 8th Avenue Line: A & C trains to Euclid Avenue; take the Crescent St. B13 bus to Linden Boulevard then walk approximately 3 long blocks East to Elderts Lane. Make a left on Elderts Lane and proceed 1 block to Dumont Avenue and you will see the nursing home. Turn right on Dumont, then left on Forbell to the entrance.

Or, take the A train to Grant Avenue, walk across the North and South Conduit, turn left, proceed to Forbell, turn right, 2 blocks to the Home. This is a 10-minute walk to the nursing home. (Entrance is on Forbell Street between Dumont and Blake avenues).
